\n\nProvides services to patients and their families to deal with needs incidental to their hospitalization. Provides counseling, support and assistance in making transition plans. Provides direction for routine aspects of non-medical problems of patients and their families. Assists with the planning and post-discharge of patients with outside agencies, nursing staff, and relatives.

\n Requirements:
Day Shift Tues - Sat. 40hrs week, 8 hrs a day, can start between 8 and 9 am.. half hour lunch.
\n\nMust have experience in social work, have an MSW standard for CA. Have acute care experience and active license.
