Job Summary and Qualifications

The Case Manager RN Wound Care is responsible for ensuring high-quality, patient-centered care through effective management of rehabilitative services. This role involves coordinating and directing the care specific to the needs of each rehab patient and collaborating closely with the Rehab Program Director and Facility Case Management Director.

Key Responsibilities:

• Generate the Daily Wound Care Report, disseminating it to inpatient units and leadership teams.
• Maintain the stock of the Wound Care Cart.
• Engage with patients alongside providers, ensuring effective dressing change and care instructions.
• Document wounds accurately, including photographs and measurements.
• Provide follow-up appointments prior to patient discharge.
• Ensure patients understand their care and have the opportunity to ask questions.
• Review patients not listed for the day's evaluation to identify any additional wound care needs.
• Prepare a daily summary for management, including a count of patient contacts and discharge arrangements.

Qualifications Required:

• Valid RN License.
