Job Title: Medicare Home Health Clinical Manager

About the Role:
We are seeking a dedicated and experienced Clinical Manager to lead our Medicare-certified home health team. This role is responsible for the day-to-day clinical oversight and management of patient care services to ensure high-quality, compliant, and patient-centered care. The Clinical Manager plays a vital role in staff supervision, care coordination, and regulatory compliance, contributing directly to the overall success and growth of our home health program.

Key Responsibilities:

• Provide clinical oversight and supervision to a team of RNs, LPNs, therapists, and other field staff.
• Ensure compliance with all Medicare, state, and agency regulations and standards.
• Monitor and manage patient care coordination, including timely and accurate OASIS assessments and plan of care development.
• Conduct case conferences and clinical team meetings to review patient progress and address care concerns.
• Review and approve clinical documentation for accuracy, completeness, and compliance.
• Serve as a clinical resource and mentor for field clinicians.
• Manage staff scheduling and productivity to ensure efficient operations and appropriate caseload distribution.

Qualifications:

• Current and active RN license in Florida
• Minimum of 2 years’ experience in Medicare-certified home health
• At least 1 year of clinical supervisory or management experience required
• Strong understanding of Medicare Conditions of Participation, OASIS documentation, and home health workflows
• Excellent organizational, communication, and leadership skills
• Proficient in EMR systems
• Ability to multitask, prioritize, and thrive in a fast-paced environment
• Must possess a valid driver’s license and reliable transportation

Pay: $90,000.00 - $95,000.00 per year
